SUMMARY OF RESPONSIBILITIES:


The Diabetes Nurse Educator- Foot Care will maximize the use of interdisciplinary resources to enable chronic disease management for the community member with diabetes. The Registered Nurse (DNE)-Foot Care will provide adult community members living with diabetes or at high-risk for diabetes and their families with knowledge and skills for self-management and the prevention of complications. In the process of self-management care and education, the Diabetes Nurse Educator will maintain a functional role in foot care assessment and high-risk identification for referral to internal and/or external foot care services. The Diabetes Nurse Educator will utilize current Canadian Diabetes Association Guidelines, Public Health and Health Promotion frameworks and the Ontario and Quebec nursing scope of practice guidelines standards to: promote health, build individual and community capacity, facilitate access and equity, demonstrate professional responsibility and adheres to Nursing Code of Ethics. The diabetes Nurse Educator will plan and implement programming to encompass a multidisciplinary team approach to diabetes education, foot care and self-management using appropriate guidance and resources.




QUALIFICATIONS:


• Proof of current registration as a Registered Nurse in good standing with the professional regulatory body in both Ontario and Quebec.


• Bachelor’s degree in Nursing from an accredited university preferred.


• Have a certificate to provide foot care.


• Credentialed as a diabetes education specialist or diabetes clinical management specialist preferred or willing to obtain certification within 2 years.


• Ability to work flexible hours, evenings, and weekends.


• Have a valid driver’s license and be able to drive MCA vehicles in accordance with Fleet Policy.
